What a wonder Fall Conference - a special thank you to all the Fall Conference Committee members and Best Meetings for pulling it off.  A recent event for MN-DONA was our Breakfast at the Care Providers convention. Coming up is the breakfast at the the Aging Services Institute.  The breakfasts are open to MN-DONA members and DONs or ADONs that are interested in becoming members.  Spring Conference is also right around the corner in April.

Thanks again to all that participated in the Fall Conference, it was so nice to see so many friends and learn some new stuff.

FALL CONFERENCE 2011

Education, scholarships, DON of the Year Award, exhibits, sponsors, networking, making new friends and visiting with your peers, relaxation, committee meetings, board meeting, prizes, MN-DONA items to purchase, silent auction (wonderful baskets from each MN-DONA District) and so much more were part of this year’s MN-DONA Fall Conference.

CONGRATULATIONS TO OUR 2011 MN-DONA NURSE ADMINISTRATOR OF THE YEAR

Each year at our annual Fall Conference, MN-DONA awards our Nurse Administrator of the Year Award to a member of our association who demonstrates outstanding leadership skills, dedication to the long-term care field and who is active in our organization. After the completion of a lengthy nomination process by administrators, co-workers and peers, the MN-DONA Board of Directors received 12 outstanding nominations for this year's award.

MN-DONA proudly recognizes all our nominees and a gives special congratulations to Jeneva Bellefeuille as the 2011 MN-DONA Nurse Administrator of Year!

Jeneva has 24 years experience as a Director of Nursing and is currently the DON at St. Otto's Care Center in Little Falls. In her nomination, her administrator said, "Jeneva has illustrated her leadership effectiveness, attentiveness to detail, ability to present information, negotiate and press important issues with integrity and fairness. There is no question the contributions Jeneva has made in long-term care have extended from St. Otto's Care Center in Central Minnesota to the four corners of Minnesota."

Jeneva has been very active in MN-DONA, having served two terms as President, Chair of the Constitution/Bylaws Committee, member of the Finance Committee and served as liaison to the MDH Pharmacy Regulation Task Force, LTC Issues Committee, and the MDH Electronic Health Record Work Group.

First I want to again say thank you for the honor of being chosen the 2011 Nurse Administrator of the year. I was very humbled and honored to receive this award.

MNDONA has been part of my life for over 15 years and I have had the opportunity to see our organization grow into the great organization it is today.
My congratulations also to the other 11 members who were nominated this year and thank you for the contributions you have made to LTC.

Every year at MN-DONA's Fall Conference, scholarships are awarded to a member or a person sponsored by a MN-DONA/LTC member, who is focusing on furthering education that will benefit long term care.

Scholarships awarded at this year's Fall Conference totaled $15,000 and included awards given to 30 nursing students from across the state of Minnesota.

SCHOLARSHIPS FOR MOLN PUBLIC POLICY INTERNSHIP

Last month, the MN-DONA Legislative Committee awarded scholarships for six members to attend the MOLN Public Policy Internship held on November 3-4. The Internship was open to one licensed nurse from each MN-DONA district who was interested in learning more about the legislative and public policy process. MN-DONA was pleased to award free registration to the following members:

The experiences of three of our scholarship winners are reflected below:

Attending the MOLN internship was a wonderful experience, and taught me many things. Upon reflection, those many things can be summed up to one main theme: Even though I am a single person, I can be responsible for the change I want to see in the future of our healthcare system.

Prior to attending the conference, I felt very removed and intimidated by the political process. I believed it would be difficult to even figure out who my political representatives were. The speakers involved in the internship really brought the political process to life, and gave a realistic picture of how to most effectively get my point across to my politician, whether by letter, by an appointment, or by running alongside my representative as he or she tries to make it to the next meeting. The speakers were able to take the massive subject of long-term care, and help me refine my views in to manageable, usable points for presenting to my politicians. Throughout the day, the ongoing theme was “You can do this. It might be intimidating at first, but it is worth it in the end. “ The speakers impressed upon the class that we are “experts” in our field, and a great resource to our politicians. Because of this, each one of us have the ability to impart useful and meaningful information, not only in our statistics and numbers, but also in our personal stories and knowledge of what happens day to day in our facilities.

In addition to discussing the political process, I really enjoyed hearing from a knowledgeable panel of educators and providers in the nursing profession. Day two of the internship had a focus on the IOM (Institute of Medicine) recommendations for nursing, which outlines eight recommendations for the future of nursing, with goals to meet by 2020. Not only was this extremely interesting, it really shed light on the importance that the nursing profession will have on healthcare over the next decade. During this discussion, I realized that 2020 is not that far in the future, and that we have a great deal of work to do to be able to meet the suggested goals. Again, I realized that I can influence change by my actions, and that each individual will play a role in accomplishing these recommendations. We, as nurses, are called to be “lifelong learners”, and need to take on this responsibility. Per one of the recommendations set forth by the IOM, the nursing profession will need to double the number of nurses with a doctorate by 2020. I need to look towards the future and see how I am going to make a difference by perhaps furthering my education and enhancing my leadership skills.

I am grateful to MNDONA for providing me with the opportunity to attend this internship. It taught me many things and especially that I, even as just one person, have a voice in what will happen in the future of healthcare. This internship was a call to action for the nursing profession, and a reminder that all we do is really for one thing: better care for our patients.

I would like to thank MNDONA and especially District V for allowing me the opportunity to attend the MOLN conference November 3-4th, 2011. It was such a positive and wonderful experience. I was able to learn so much about the legislative process and more importantly my role in public policy. I am excited to continue learning more about the legislative process and also becoming more involved. I learned that I can make a difference and that every voice counts; that was powerful. I also had the opportunity to network with other nursing professionals and learn from them also. I am very lucky to be a member of MNDONA and value the experiences that have been given me through being a member.

I was very impressed by the speakers and the tour of the capitol.  The panel discussion on Friday was the highlight of the experience. It was nice to have a group of nurses together from many different avenues of nursing discussing the challenges of acute care, long term care and nursing practice in the very near future.  

I have been at the capitol for numerous rallies with Aging Services and I have written letters before to our legislators, but this internship was very beneficial on reviewing the whole process and how to advocate for long term care needs. I t was a very worthwhile two days away from the home!

LEADERSHIP DEVELOPMENT/PERSONAL BALANCE SCHOLARSHIPS

The purpose of the MN-DONA Leadership Development/Personal Balance scholarship is to further the education or personal well-being of a MN-DONA member who is currently a DON or ADON.

CRITERIA TO BE ELIGIBLE FOR AND RECEIVE A MN-DONA SCHOLARSHIP AWARD:
• Applicant must be seeking enrichment in personal or professional arena.
• Money must be used for education fees or personal balance activity.
• Applicant must be a current member of MN-DONA and currently employed in long-term care as a DON/ADON.
• Recipient may re-apply on a yearly basis.
• Please explain in a paragraph how you would use the money for a leadership/personal balance activity.
• Recipient should submit a follow-up article (within two months of the event) about the personal/professional enrichment for inclusion in the chapter newsletter, "The MN-DONA Focus."

APPLICATION PROCESS AND SELECTION IS AS FOLLOWS:
Complete the scholarship application form available on the MN-DONA web site at
www.mndona.org, along with a paragraph explaining how you would use the money for a leadership/personal balance activity, and submit to the MN-DONA Scholarship/Education Committee by February 17, 2012. Applications received after this date cannot be considered. Attach a copy of the informational brochure for which the money will be used.

MN-DONA'S TMA POSITION PAPER

MN-DONA’s Position
The Role of the T.M.A. in Long Term Care

Purpose
To give direction and support to the Director of Nursing (DON) who utilizes a Trained Medication Aide (T.M.A.) in providing delegated medical care in the long term care (LTC) setting.
Issue
  1. Minnesota has no guidelines for DONs to reference when making a decision to utilize the TMA in their facility.

  2. LTC facilities throughout Minnesota utilize the TMA in a wide variety of scope of practices.

  3. There are no educational guidelines for maintaining skill/competency of the TMA in long term care.

  4. The reality of resources and workforce for licensed staff are challenged both financially and demographically.
Position
  1. MN-DONA accepts and supports the use of the TMA in LTC for the delegated task of routine medication/treatment delivery.

  2. The TMA must be 18 years of age or older, have a current Certification as a nursing assistant and in good standing with the MDH registry, must have a certificate of completion for a TMA training program, must be recommended by the facilities DON.

  3. The delegation of the administration of medication/treatment must be monitored by a licensed nurse, and delegated by the Registered nurse.

  4. The DON is accountable to assure the TMA who delivers medications and treatments under his/her license has ongoing evaluation and training.

  5. The TMA will not:

  6. Convert or calculate medication dosages

  7. Assess need for or response to medication, other than collecting data for the licensed nurse

  8. Judge the need for a PRN medication

  9. Give medications via parental, nasagastric, gastrostomy, or jejunostomy routes.
Summary
MN-DONA respects all roles in the long term care setting, professional, licensed, and non-licensed. There is no substitute for professional nursing judgment and roles are not interchangeable.

It is critical that Directors of Nursing feel safe in their practice. Directors of Nursing require the support and flexibility to delegate nursing functions to the T.M.A. MN-DONA encourages regulatory agencies to work with the Directors of Nursing in facing this challenge and issues of declining reimbursement, regulatory expectations, and most importantly the crisis of limited human resources facing the health care industry.

DISTRICT I NEWS!

October 12, 2011 District one met at the MNDONA fall conference.

On November 10, 2011 we had another meeting at Pioneer Care Center in Erskine. We had 10 that were able to attend. Jon Langlois and Mary from McKesson did a presentation on their new skin care line. They had a wonderful meal catered for us. We discussed the most recent survey citations, staffing, and other issues impacting our facilities.

We will meet again the 2nd Thursday of January 2012 at 12:30.

DISTRICT II NEWS!

District 2 continues to meet monthly on the third Friday of the month at Blackwoods in Proctor, 12:30pm – 2pm; lunch is on your own.   Average attendance is 5-7. We encourage all district members to attend as we have great discussions on surveys, staffing, the latest guidelines and what we are doing about them, we have a chance to share best practices, offer support, and gain insight to our difficult challenges.

DISTRICT III NEWS!

District III last met at Fall Conference on 10/12/11. Here are the topics we discussed:
1 - MOLN: All were informed of a sponsorship by MN-DONA for the registration cost to the MOLN Public Policy Internship November 3-4. Each district is invited to
send one member. Kari Armstrong, DON at Glenwood Retirement Village, is interested
in attending from District III.
2 - Recent surveys: Much discussion on the recent number of G tags given. Most stem from ROM; either no plan in place or plan not followed causing harm. Also discussion on if the staff knows the definition of supervision as a facility received a G level tag on a res. that fell in front of a CNA who was providing supervision with ambulation. Another facility received a tag for unsupervised steam table and hot coffee pots that wandering residents could touch. A facility in Duluth had a resident elopement. The resident was found by a staff member on her way to work in the early am. She picked him up and brought him back to the facility and did not tell anyone for a week. They reported once they knew and received a tag when OHFC investigated. There are 4 new OHFC investigators. All in attendance agreed we learn tremendously from other facility surveys and are grateful for the sharing.

DISTRICT IV NEWS!

Meeting minutes: 11-2-11
Discussion included the following topics: MDH November 7 conference call on abuse that will cover the elder justice act; Survey – focus continues on contractures, rehab nursing, VA.

Kelly from the St Cloud Hospital Behavioral Health Unit did a presentation on how the ER assesses residents for suicidal ideation. SCH has 20 beds of which at least 75% of the time they are full. She suggested asking the intent of the patient, psych hx, what’s going on with patient, how long they have been thinking about suicide, med compliance, and any recent losses.

Roundtable discussion included geriatric service MDs dx “hospital acquired delirium” more frequently; this is under diagnosed according to the Medical Director conference, they aren’t treating it, rather saying it’s environmental r/t hospitalization; discussed mandating employees during staffing crisis; the St. Cloud Times was going to publish OHFC reports.

District V North Group met 11/17/2011 at Rice Care Center.  A tour was given of their new Therapy Suites that opened August 1, 2011.  Nine members were present.  Becky Hughes, Director of Nurses at Clara City Care Center attended MOLN Internship on November 3-4th for District V.  She gave a brief report of her experience and learning that took place.  Reminder to those present to mark your calendar for Spring Conference in April 2012. Report from those that had recent surveys in the area.  Round Robin for sharing what is going in the buildings etc.

On my last count there were 55 District VII members in attendance this fall.  We had a brief meeting on Wednesday October 12 with 17 members. 

Other news from the district-

  • Kija Robberstad, from Walker Methodist, will be attending the MOLN Internship on Public Policy November 3rd and 4th sponsored by MN-DONA.  This is an exciting opportunity to learn about public policy and to develop skills for advocacy.  I look forward to hearing about her experience.
Jolene McDonald will be coordinating an area meeting on November 10th.
Winter Blast coming soon to Shakopee – the group agreed to hold a District VII meeting in January at Saint Gertrude's.  I will be sending out more information on the date and time later this month.  If you have some suggested topics or speakers please send them to me @

TMA position paper - This has been completed and was available as a handout in the Fall Conference attendee packet and is on the MN-DONA web site.

MN-DONA Day at the Capitol: Jacqueline Jenkins will draft an informal survey about combining with Aging Services or Care Providers. We will check with the MN-DONA Board to see about an online survey to get a better feel. Here are some sample survey questions:

  • Should we combine with Aging services or Care Providers?
  • Would you go?
  • If no, what would prevent you from going?
  • How much advance notice do you need?

NOMINATING COMMITTEE

In 2012 there will be five offices on the MN-DONA Board that will be up for election. We would like you to consider running for one of the following offices or nominate someone you believe would be a good representative for MN-DONA:

Recording Secretary
This position involves accurately recording the minutes of all meetings and presenting the same at each meeting. Minutes are to be forwarded to the President within 30 days of the meeting that was recorded, to be forwarded to Committee members for clarification and/or correction. The Recording Secretary is a member of the Executive Committee by virtue of the office. The term of office for this position is two years, elected in even years and is effective at the annual meeting.

Corresponding Secretary
This position involves being responsible for the dissemination of any and all correspondence including the Newsletter of the organization as determined by the President or Executive Committee. The Corresponding Secretary is also a member of the Executive Committee and the terms of office shall be for two years, elected in even years. The term is effective at the annual meeting.

District Directors
The positions for the even numbered districts of 2, 4, and 6 are open for nominations.

The Committee of District Directors consists of one person from each of the seven districts in Minnesota. The District Directors may appoint Area Coordinators, as required, to facilitate communication and development. Area Coordinators will not be included as members of the Executive Committee. The District Directors may authorize a proxy in writing to represent him/her at an Executive Committee meeting. The proxy has the ability to vote as needed. No District Director may hold more than one office concurrently.

The District Director shall:

  • Assist the district with the organization and development of a local organization of Directors of Nursing in Long Term Care.
  • Act as a liaison between the Area Coordinators and local members residing in his/her district with the Executive Committee.
  • Shall report to the President on a quarterly basis and supply proposed agendas and any assessed fees for meeting for Executive Committee approval.
  • By virtue of his/her office, the District Directors shall be members of the Executive Committee and be actively employed as a DON/ADON. Should the employment status change, a one-year transition period will be allowed.
  • Be elected by mail ballot done concurrently with the election of officers. Terms of office shall be two years. No member shall be eligible to serve more than two consecutive terms as District Director. Odd numbered districts shall elect a District Director on odd years and even numbered districts shall elect a District Director in even number years. The election of District Directors is done by members of the District only, not the membership at large.
  • If a District Director position is vacated during the term, the President along with the Executive Committee shall appoint a replacement from that District to complete the term of District Director.

Membership:

  • Discussed current membership of 293 and the goal to reach 300; won’t give away the electronic reader due to not meeting our goal but will give away a ‘Kindle Fire’ if meeting 300 goal at Spring Conference. We did gain 11 members!
  • Discussed adding MN-DONA to district meeting agendas for Aging Services/ Care Providers.
  • Discussed asking to put out information about MN-DONA/ folders with colored sign up/ more information sheets at the Aging Services and Care Provider conferences.
  • Discussed if we should have discounted membership dues for Spring Conference, similar to Fall Conference
Merchandise:
  • The previous coffee mugs leaked so they got returned
  • The hot new items are the water cups for $10 and USB drives for $15
  • Do we need more of the bigger portfolios - need to check supply

  • Discussion on adding a ‘count down to conference’ and/or a countdown to the membership drive

Scholarship Committee Meeting 10/12/11 (at Fall Conference)
 
 
DON Certification
The MN-DONA Board shared concerns of a certification with NA-DONA due to past history with the NA-DONA organization. The Board has requested that the committee research an alternative choice for certification
 
Options will be researched by committee members and forwarded to Wendy Lonetto, Committee Chair. Wendy will summarize the information and forward it to the MN-DONA Board.
 
Discussion
American Association for Long Term Care Nursing
20 hour on line course, Certification questionable.
 
The Board requests that we explore the NA-DONA certification if there is nothing else available. Cindy Fronning continues to be a NA-DONA member. She reports that the organization has resolved the issues legally.
 
When researching an alternative certification the committee will want to know

  1. Organization
  2. Certification
  3. What makes it available
  4. Renewal requirements
  5. Cost
  6. How do you maintain the certification.
 
Scholarships
The committee determined that all members are not using the same qualifications regarding the letter of acceptance. There have been some applications sent in with a report of classes being taken. Going forward the committee agrees that it will only accept the acceptance letter into the nursing program. The committee agreed to communicate to the membership not to encourage students to apply unless they are currently in a nursing program.

MN MEDICAL DIRECTORS ASSOCIATION

Report on MMDA Conference on October 17 and 18th, 2011:
 
Dr. James Pacala, MD, MS, and Dr. Edward Ratner, MD, spoke on the use of medical students taking course work on the elderly and the ethics of Adult End of Life Care. 

Dr. Tracy A. Tomac, MD, spoke on Psychiatric Medication Reduction in the elderly.  She recommended the use of the clock test for cognitive testing.  It gives a better reading on the cognitive ability of the elderly. 

Dr. Sontag spoke on assessment and critical thinking that is so important for the nurse before calling the MD.
 
I would like to comment that this conference is not just for Medical Directors.  It has valuable information to share regarding our work in LTC.

DHS RELEASES RFP

2012 PIPP Team Workshops
Sponsored by the Minnesota Department of Human Services
Nursing Facility Rates and Policy Division

This year there are exciting changes in the Performance-based Incentive Payment Program (PIPP) and we encourage you to take advantage of one of our free Team Workshops. Workshops will be a combined lecture format and a one-hour break-out session to allow teams to begin planning and utilize the guidance of the technical advisors. These workshops are great introduction to PIPP and also serve to update experienced facilities about this year’s new developments.

Participants: Nursing home staff, central office staff and long term care consultants.

Presentation topics (2 hours)

  • Introduction and brief overview of PIPP
Information about the new newsletter, PIPP Quarterly Update, and a networking website being developed
Details of the new Mentoring Program opportunity
Step-by-step demonstration of the new electronic PIPP 2012 Application Form and Instruction Manual including instruction on:
    • How to use the new and improved rate calculator and submit a required budget estimate.
    • Increased auditing and sustainability plan requirements
  • Instruction on submitting the new Six Month Status and Final Reports
  • Discussion of performance measures including the Interim Minnesota Quality Indicators and the University of Minnesota’s recommendations for new Minnesota QIs

Team breakout session (1 hour)

Internet access will be available and each team is encouraged to bring a laptop if available. The technical advisors will also have laptops to assist you. We encourage you to bring data that will help you identify an area for improvement. Data would include but does not have to be limited to the Minnesota Quality of Life Resident Satisfaction Survey and/or the Minnesota Quality indicators. If a team has already chosen an area for improvement, they should bring the data that supports it. Individual technical assistance and review of any prepared draft materials will be provided. The technical advisors will be available to help each team with:

  • Using your facility’s data to identify an opportunity for improvement
  • Ideas and tools for discovering root causes
  • Developing a strategy and work plan
  • Suggestions for evidence-based resources can be used to develop and support strategies
  • Finding outcome measures that will adequately capture the intended effect of the strategy
  • Determining a proposed rate increase that aligns with the scope and complexity of the project
  • Building a leadership plan to carry out the strategies and insure sustainability
  • Establishing a plan to audit and report progress
